Are there any issues of oil creeping down into the cylinders when you throw today's small 4-stroke outboards 6 hp or less into the back of a pickup truck for transportation? I never thought of this issue since I've used only small 2-strokes.

YES!

The OEM manual will tell you how and HOW NOT TO STORE it ...

FWIW my bro bought a new 2hp 4S Honda many years ago, left on the dinghy that was pulled up on the dock. If he turned the OB one way (to keep the prop away from people’s feet) it would start ... but if he turned it all the way the other way, it wouldn’t restart at all. And that was only stored w/ the OB at a 45-degree angle.

Yeah it makes a difference. Read the manual for the motor and it'll tell you which way not to do it.

It's says right on my 6hp 4 stroke to lay the motor down with the shifter lever up. No issues as long as you do that.
